Advance directives are instructions that have been prepared in advance that are intended to direct a person's medical care if he or she becomes unable to do so. Five Wishes is both a Living Will and Health Care Power of Attorney in one.

Written with the help of The American Bar Association's Commission on Law and Aging, the Five Wishes program is designed to comfort those who may fall seriously ill as well as their family members during trying times. This will meets the legal requirements for care at the end of life and is the first living will that discusses personal, spiritual, and emotional needs in addition to medical needs.
The program is for anyone 18 or older-married, single, parents, and adult children-and has already been used by more than 13 million Americans.
After signing the Five Wishes form, it will replace any prior advance directive of the patient. The "wishes" within this program allow families and doctors to know:
- Who you want to make medical decisions for you when you are unable to do so
- The kind of medical treatment you want or don't want
- How comfortable you want to be
- How you want people to treat you
- What you want your loved ones to know
